Create forms with Conditional Logic that can show or hide fields based on user response.
 
The example below has four credit card related conditional fields all tied to one check box.

Conditional fields can be set up to be triggered by:
a. Any entry into a text field.
b. The entry into a text field is equal to a word, phrase or numeral selected by you.
c. The entry into a text field is not equal to a word, phrase or numeral selected by you.
d. The entry into a 'numbers only' text field is less than a numeric value selected by you.
e. The entry into a 'numbers only' text field is less than or equals to than a numeric value selected by you.
f. The entry into a 'numbers only' text field is greater than a numeric value selected by you.
g. The entry into a 'numbers only' text field is greater than or equals to than a numeric value selected by you.
h. Any selection of a drop down box or radio button.
i. The selection of a drop down box or radio button is equal to a word, phrase or numeral selected by you.
j. The selection of a drop down box or radio button is not equal to a word, phrase or numeral selected by you.
k. A click of a checkbox. (shown in the example above)
