I hereby attest that the information provided in this application is true, accurate and correct. Cornerstone Realty Management Corp. is authorized to obtain consumer reports, and any other information it deems necessary, for the purpose of evaluating my application. I understand that such information may include, but is not limited to, credit history, civil and criminal information, records and/or any arrest, rental history, employment/salary details, vehicle records, licensing records, and/or any other necessary information. I/we understand that subsequent consumer reports may be obtained and utilized under this authorization in connection with an update, renewal, extension or collection with respect or in connection with the rental or lease of a residence for which application is made. Applicant has received, read and understands the form titled ``Summary of Rental Criteria`` prior to making an earnest money deposit. I/we hereby attest that the information provided is true accurate and correct. I/we understand the $30.00 application fee is non-refundable and I hereby deposit $200.00 as earnest money, which will be retained and converted to a security deposit upon taking occupancy of the Premises. I/we understand that the application fee, if any and/or earnest money deposit will be forfeited as liquidated damages resulting from the misrepresentations or omissions of facts or my refusal to sign a lease and take possession after I have been approved to rent an apartment. Upon acceptance, I/we will be signing a one-year lease, the application fee, if any, will not be applied toward the security deposit. I/we understand and agree that any credit granted by Cornerstone shall be paid promptly in accordance with the terms and agreements, that the credit grantor may add late charges to any balance owed, and in the event application is withdrawn prior to acceptance or in the event of applicant rejection the earnest money deposit shall be returned.

 

I hereby authorize Cornerstone Realty Management Corp. to charge my credit card, using the information that I have provided, for the required application fees and earnest money deposit. Further, cardmember acknowledges receipt of goods and/or services in the amount of the total shown hereon and agrees to perform the obligations set forth by the cardmember`s agreement with the issuer.
