All requests must be made between August 1 and November 1 by the local ensemble chair only.

Two choices should be submitted for each ensemble. Submit choices for only one ensemble per form.

Eligible number of ensembles is based on last year`s SA enrollment. Total minutes of performance must include time for entering and exiting stage.

Make sure your email address is entered correctly below. Upon successful submission of this form, an email receipt will be sent to this address. Instructions for sending the first page of the primo part of each ensemble choice will be included in this receipt. Repertoire approval will not be final until the Repertoire Assistant has received these copies.

 

If mailing copies for approval, send 1 copy of this form for each ensemble entered. Send a copy of the first page of the primo part for each ensemble. Approval verification will be emailed.

If emailing entries, attach image file(s)of the 1st page of the primo for each ensemble to an email to the HS/YA Repertoire chair at lozanotrio@msn.com. Use 1 file for each ensemble performing at convention. Approval verification will be emailed.
