Award Category
Nominees need to prepare a document containing the following information and submit it to the nominating association`s president. The document should be a one-page 8 1/2 by 11 inches, single-spaced document with one-inch margins, typed in 12-point Times New Roman font. The document may be saved as MS Word, pdf, or Rich Text format.

Include as much or as little information per category as you wish. Documentation may not exceed the specified one-page limit.
